Insurance and Billing

Family Medical Center maintains a list of health care plans with which we have contracted to provide services to our patients. We have agreed to bill those insurance carriers for all services rendered. This, however, does not allways guarantee payment. Therefore, if the insurance company does not pay within 60 days, claims follow up and payment for services becomes the responsibility of the patient.

If you have an insurance plan with which we are not contracted, you will be responsible for payment at the time services are rendered. We will provide you with a receipt acceptable by your insurance company.

We do not accept Automobile, Liability, or Worker's Compensation Insurance. If the reason for your visit is a result of an automobile accident or worker's comp claim, you will be responsible for payment for services at the time of your visit.

INSURANCES WE ACCEPT

Currently we participate with the following insurance plans:

VERIFICATION OF INSURANCE

The day before your visit our insurance specialist will verify your coverage and benefits. If it is discovered that you are not covered for whatever reason, you will be responsible for payment of services at the time of your visit.

If your insurance has changed since your last visit, please notify the receptionist when you call to make your next appointment, or immediately upon your arrival in the office.

It is imperative that you have an insurance card which shows the effective date of coverage, and if required by your plan, designation of one of our physicians as your Primary Care Physician (PCP). If you do not have these items, you will either have to reschedule your visit or be responsible for payment for services until you have proof of insurance and PCP designation.

CO-PAYMENTS AND DEDUCTIBLES

Co-pay and deductibles are due at time of service. If you are unsure of these amounts, please contact your insurance company prior to your visit.

NON-COVERED SERVICES

There may be services you receive that are not covered by Medicare or your insurance plan. We will advise you before you receive that service that there is a possibility that it might not be covered. You will be asked to sign a statement verifying that you will be responsible for the associated charges should Medicare or your plan not cover them for whatever reason.

Specialist Referral Guidelines

  1. The physician or P.A. may refer to to a specialist

  2. If you are instructed to make your own appointment, please call the specialist's office and make your appointment for no sooner than 1 week away, allowing our referral department enough time to obtain an authorization from your insurance company.

  3. After you make your appointment you must call Lautrice, our Referral Coordinator at 407.992.7745, and leave the following detailed message:

    1. Specialist's name

    2. Date and time of your appointment

    3. Your name

    4. Your date of birth

    5. Your insurance company ID# (usually the social security number of the insured).

  4. Lautrice will obtain the authorization for your visit and notify the specialist office.

  5. Lautrice will notify you that the authorization has been done.

IMPORTANT POINTS

  1. Lautrice needs 1 week maximim to complete the authorization process. Therefore, make sure you allow that much time when you schedule your appointment.

  2. If you do not follow these guidelines, you will need to reschedule your appointment until the authorization process is completed, or you will be responsible for payment for the specialist visit.

Laboratory and X-ray Testing

SCOPE OF SERVICES

LABORATORY

Our office performs lab work for testing ordered by Family Medical Center Physicians only. We will not honor prescriptions for lab work from other physicians, such as a specialist you may be seeing. In those cases, you must go t the reference lab (Quest or LabCorp, for example) that is designated by your insurance company.

For lab work ordered here, our office performs a limited number of "stat" labe tests "in-house." For the majority of more complex testing, our lab technicians will draw your blood and send it to the reference lab for testing.

Please Note: You may have an insurance company (Humana, for example), that requires you to go to the outside lab to have your blood drawn. In this case, our lab technicians will give you an order form that you will take to the referenced lab draw station with map provided.

X-RAY

We perform most routine x-rays and Dexascans (for osteoporosis) ordered by our physicians in our office.

Please Note: You may have an insurance company, (Aetna, for example), that requires you to go to an outside facility for your x-ray. In this case, our referral coordinator, x-ray tech or the nurse will direct you to the appropriate facility.

TEST RESULTS

LABORATORY

If you have a follow-up appointment with the doctor or PA, your lab results will be reviewed at that time. If you do not have a follow-up appointment, you may expect notification in the following manner:

  1. Normal results will be reported to you via phone call or post card within 10 days of your test,
    or

  2. If your results are abnormal, you should expect to receive a phone call from the nurse, doctor or PA within 2-3 days of your test.

X-RAY

If you have a routine x-ray in our office, you will receive your results the same day. If you are here for a pre-physical chest x-ray, you will receive those results the day of your physical exam.

If required by your insurance company, you have ben referred to an outside facility for your x-ray, your results will be available to our office within 5 business days. You will be contacted by phone by our office when we receive those results.

Notification of Dexascan Results
After you have your Dexascan, you should schedule a follow-up appointment to discuss the results with the doctor or PA.

If you need to take your x-rays to a specialist, please allow 3 business days for our office to prepare your x-rays for pick-up.

Diagnostic Test Referral Guidelines

  1. The physician or PA has requested that you have a diagnostic test at a facility outside of our office.

  2. You must take a written prescription for the test with you when you go to the Diagnostic Facility Hospital. You may obtain the prescription in one of the following two ways:

    1. The physician or PA will give you the prescription during your visit here, or

    2. If the nurse has notified you by phone that you need a test, you must come by the office to pick up the prescription.

  3. The physician or P. will instruct you whether you should call to make the appointment or whether the nurse will do that for you. If you are not clear about this, PLEASE ASK! If you are making your own appointment, please schedule it no sooner than one week away, allowing enough time for our referral department to obtain an authorization as described below.

  4. Some, but not all, tests require an authorization from your insurance company. Our Referral Coordinator, Lautrice, or your nurse will obtain this for you. Lautrice will then call the diagnostic facility who needs this authorization before the test can be performed.

PLEASE REMEMBER

  1. IF YOU DO NOT HAVE YOUR PRESCRIPTION WITH YOU AT THE TIME OF YOUR SCHEDULED TEST, YOU WILL HAVE TO RESCHEDULE.

  2. SHOULD YOU LOSE THE ORIGINAL PRESCRIPTION, OUR OFFICE WILL NOT BE ABLE TO PROVIDE YOU A DUPLICATE AT THE LAST MINUTE.

  3. THESE ARE THE REQUIREMENTS OF YOUR INSURANCE PLAN.
